fix(chat): keep confirmed final answers visible after continuations - #27991
A final assistant answer moves inside Worked for when a notification or steer continues the same run and another assistant message arrives. The timeline currently protects only the last message per run.
In the reported Claude incident, the original SDK text snapshot had a null stop reason, but a matching native message received end_turn 14 ms later. The saved transcript independently confirms it. That signal never reaches the client fold.
Change
Retain optional answer phase metadata in assistant turn items and both storage/wire codecs. Codex confirms explicit final intent only after native item completion, so cleanup cannot promote interrupted text. Late completion on a retained turn still promotes the existing item after cleanup. Claude associates the final stop reason with the text blocks of the matching native message, including delayed metadata and successful result-only responses.
Web/desktop and mobile keep every completed, non-streaming confirmed final answer visible, including across superseded attempts. Explicit commentary folds. Terminal footer selection remains separate from visibility, and old history or unsupported provider paths keep their existing fallback. There is no database schema migration or historical backfill.
Scope and approval
Addresses the provider-confirmed-answer case reported in #8879. The narrower metadata-based failure and source evidence are recorded in this report; the fork report contains sanitized UI evidence.
This is a focused repair of the existing final-answer presentation rule: preserve provider-supplied answer intent instead of discarding it and choosing by position. The contract, two adapters, and client predicates are all necessary to fix that one defect. It adds no setting or workflow, and preserves progress folding and the fallback where intent is unknown. This is submitted under the focused obvious-bug exception; it does not implement the broader keep-all-assistant-prose change in #14698.
Verification
Regression assertions fail on the unchanged implementation: missing provider phases, stripped codec fields, and earlier final answers hidden by both clients.
Focused suites: web timeline 93, mobile timeline 45, contracts 15, and Codex adapter 127 tests pass. Claude metadata selection 8 passes, covering snapshot and delayed stop reasons, block association, nested-message isolation, and result-only fallback. Added Codex regressions fail before the corrections and pass after them: completed and interrupted turns without native item completion cannot confirm unfinished text; a retained turn's late completion promotes the same item after cleanup.
Final vp run check:changed --concurrency 2: format, lint, both knip checks, GraphQL generation, all 35 affected package typechecks, and React Compiler pass. The test step reports 549 passes, 13 failures, and one existing skip. All 13 failures reproduce against the byte-identical upstream Claude adapter on Windows: attachment-path escaping and the tool-search setting case. No expectations were weakened or tests suppressed.
All upstream checks passed on 31027594ad4ccc74a4f6df15a0bf7976a77169be, including lint, all server shards, web tests, tests, build/typecheck and release smoke. The separate exact-commit fork run completed with all check jobs passing except an unrelated Server tests 2 failure: storageCleanup.test.ts:1974 expects an invalid worktree gitfile to be kept, but Git 2.56.0 returns gitfile does not point to a valid repository, which upstream storageCleanup.ts does not classify as a missing repository. Both cleanup files are unchanged by this PR. The personal fork already carries handling for that diagnostic; this upstream-based branch does not include that independent fix. No unrelated code or test suppression was added here.
Real development web client: the same saved synthetic fixture on upstream and the patch, with two confirmed answers, notification, tool activity and trailing commentary. Upstream hides both answers; the patch shows both with work collapsed. Expanding and collapsing restores/hides commentary while both answers remain visible; reload preserves the result. Fixtures were written only to stopped sandbox projections and do not stand in for adapter protocol tests.
Native mobile and packaged Electron were not exercised. This Windows machine has no Android SDK or iOS simulator. Desktop uses the changed web timeline; mobile has focused logic coverage.
Independent GPT-6.1 Sol source review returned no actionable findings. Later integration review found the interrupted Codex-text edge case; it is corrected, covered by the two regression cases, and independently re-reviewed clean. No live database or running-app changes.

Walkthrough
Assistant turn items now support optional final_answer and commentary phases. Claude and Codex adapters assign phase metadata from provider events. Web and mobile folding use the metadata to retain confirmed final answers and fold commentary or other trailing activity.
The adapters attach phases to assistant-message items using provider metadata. Tests cover snapshots, stream events, and messages without recognized phase information.
Web and mobile folding preserve confirmed final answers and apply phase-aware rules to commentary and trailing assistant messages. Tests cover collapsed and expanded timelines, interrupted answers, and superseded attempts.

Full details: Description check
Explanation
The description is detailed and maps well to the template. It has a Problem section that explains the bug, reproduction context (Claude incident, 14 ms delayed end_turn), and expected behavior. It has a Change section that explains the fix across contracts, both adapters, and the web and mobile clients. It has a Scope and approval section that links issue #8879 and the fork report and explains why the focused obvious-bug exception applies. It has a Verification section with specific suite counts, before/after regression evidence, a real web client comparison, and screenshots. It also names the model and harness that did the work, as the template requests. Minor gaps: the Verification section reports 13 failing tests on Windows and a separate Server tests 2 failure, and it attributes them to unrelated pre-existing issues; the author supports that with reproduction against upstream. The description is long and contains some content that is more status log than description, such as the current-head verification block. The required sections are all present and substantively filled in, so the check passes.

Reproduced on github/main (54c5f25249961a1aa0b61932329092eadbd9d6fc) in a separate worktree after vp i. An active Codex goal emitted a final answer, automatically continued in the same run, emitted commentary, ran git status --short, and emitted a second final answer. Once settled, main hid the first answer inside "Worked for".
Stopped main before starting the exact PR head after its own vp i. Repeated the identical prompt with fresh real turns. Both final answers remain visible with work collapsed. Expanding shows commentary and the command; collapsing hides those while retaining both answers. Reload preserves this. A subsequent normal turn still folds progress, shows its final answer, and retains the two earlier answers after reload.

Live Claude metadata and delayed stop-reason behavior: Claude is not authenticated on this VM. Its existing adapter suite passed, which does not establish live integration.
Native iOS/Android and packaged Electron, including macOS/Windows behavior: exercised the Linux web client only.
The original Claude notification/steer incident and superseded-attempt variants were not reproduced live. The real Codex goal continuation established the same-run final-answer preservation path; the initial delegated-child notification used a separate run and did not reproduce the bug.
Keep native final-answer intent pending until Codex completes the message.
Turn cleanup may flush unfinished text as completed, so it must retain
commentary behavior rather than qualify for final-answer visibility.
Cover both interrupted and completed turns that omit item completion.

Added 56549eb3d8 after integration review found an interruption edge case: Codex labels a final-answer item at start, while turn cleanup can flush unfinished text as completed. The adapter now retains that intent as pending until native item completion, preventing truncated text from receiving the final-answer visibility exemption. Completed answers still retain their metadata through cleanup and image rewrites.
Two regression cases failed before the correction and pass afterward, covering interrupted and completed turns without item completion. Independent correction review is clean. Local changed-file checks pass all static checks and all 35 affected package typechecks; selected tests report 548 passes and the same 13 Windows Claude attachment/settings failures previously reproduced on the unchanged adapter. The integrated fork's full Codex suite passes all 159 cases.
The PR description now records the correction and current validation. Maria's real-turn verification and the earlier green checks remain evidence for the preceding head; CI is running again for this update. No production database or app changes.

Do not create a fold for an empty leading segment.
If a confirmed final answer is the only output before a steer and commentary follows the steer, this line preserves the answer and hides only the later commentary. deriveTurnFolds still emits a “Worked for” fold before the answer with no hidden entries. Emit the leading fold only when its segment hides an entry; keep the later steer fold. (raw.githubusercontent.com)
