fix(server): keep attachments while forks still show them - #26087
Deleting a source thread removes files that its forks still display. This extends the page-lifetime fix to message and question-answer attachments, keeping each file until the last live thread that can show it is deleted. Files after a fork's cutoff still go with their source.
Selection follows each fork boundary through deleted ancestors, matches imported runless history, and excludes side chats from inherited history. Fork creation and deletion share the existing lock through commit. Atomic parent/side-chat deletion excludes the whole deletion set from surviving readers. Transcript reads use the family thread indexes even when SQLite has no planner statistics.
Final head 844c0e3ab436fde268786fa68dce5e5a4a9cc7d8, base cefb544af3c98224874419198c0359b652ca3c94. Physical corex, Linux x86_64, Node 26.12.1, Vite+ 1.4.0, isolated worktree state, real web client and development server, 2026-11-03.
Before, main cefb544af3c: fork a reply, delete the source, reopen the fork. The image is broken and its file is gone. Recording at 3x; the screenshot captures the broken image after reopening.
before-3x.mp4
After, final head: fork, delete the source, fully stop/restart the server and web app, reopen the fork. The blue image loads with a fresh HTTP 200 and the notes file opens in the attachment viewer. The generated page case keeps the inherited page and removes the later page. Deleting the final forks removes all three retained files. Recording at 3x, no steps cut; includes the restarts and final deletions.
final-after-3x.mp4
The message case uses the free Replay provider. Its first image-bearing turn is rejected because Replay does not support image prompts; a second text-only replay completes the reply used for forking. The attachments are persisted by the real client/server. The page fixture creates completed runs and pages through EventSink, replacing generation only. Forking, deletion, history reads, page loads and filesystem cleanup use production paths. Only isolated test state was used, with no paid provider.
Validation
vp run check:changed passes format, lint, knip, GraphQL generation, three affected typechecks and 218 tests. A focused eight-file run passes 205 tests. The four fork-files regression rows cover nested cutoffs, deletion orders, native/imported runless history, answer files and shared IDs in atomic side-chat deletion. They fail before the fix and pass after it. Existing runtime tests cover overlapping deletion, fork/delete ordering and project deletion. A 50k-message/50k-item probe confirms indexed transcript reads without planner statistics. Independent Codex R1 findings are fixed; fresh R2 is clear.

# Independent R2 review
Result: both R1 findings addressed. No unresolved blocker, should, or nit findings in the requested fix scope.
Reviewed candidate `844c0e3ab436fde268786fa68dce5e5a4a9cc7d8`, only its changes since R1 head `8a414ed610c96a3d6c5d37dea9b243e9a6f66981` and the remaining R1 findings. Main baseline is `cefb544af3c98224874419198c0359b652ca3c94`. The shared worktree was clean at the candidate before and after review. This review session did not implement the code.
Environment: existing Linux worktree `/home/shivam/Developer/.state/issue-watch/worktrees/30017`, Node `v26.12.1`, Vite+ CLI `0.3.2`, local Vite+ `1.4.0`, Vitest `5.0.5`. Tests used the source checkout and isolated in-memory SQLite fixtures, with no application build or client started.
Read `review-r1.md`, `review-fixes.md`, repo AGENTS/CLAUDE and testing guidance, and Macroscope approvability, ignore, and check-agent guidance. No production/test edits, pushes, posts, servers, browsers, devices, or delegation.
## R1-1: should, addressed
`ProjectionStore.ts:5558`, `5572`, and `5593` use `FROM family CROSS JOIN` for message attachments, question-answer attachments, and dynamic-tool page candidates. The join conditions still constrain each transcript read by the family member's `thread_id`.
Independently reran `node /tmp/t3code-30017/review-fixes-probe.mjs` against the final source. Exit 0; output is `review-r2-probe.log`. With 1,002 threads, 50,002 messages, and 50,000 turn items, both never-analyzed plans and plans after `ANALYZE; DELETE FROM sqlite_stat1; ANALYZE sqlite_schema;` search the transcript indexes by `thread_id`. This matches the statistics policy in `Sqlite.ts:37-42`.
- Message query: `SEARCH message USING INDEX orchestration_v2_projection_messages_thread_created_idx (thread_id=?)`.
- Both turn-item queries: `SEARCH item USING INDEX orchestration_v2_projection_turn_items_thread_run_idx (thread_id=?)`.
- No `SCAN message` or `SCAN item` in either statistics state. JSON virtual-table scans expand the selected family rows only.
The pre-existing threads-table family scan remains, as explicitly excluded from this review.
## R1-2: should, addressed
`Orchestrator.ts:13117` passes `[command.threadId, ...sideChatIds]` to every deletion plan, including each side chat's plan. The outer `command` remains the parent command; the per-plan `deletion.threadId` selects that plan's own visible candidates. The exact same parent plus side-chat list supplies the plans at `13138-13150`, whose events and effects are returned together. The existing dispatch wrapper acquires the parent, side-chat, and shared deletion locks before planning. No unrelated family member is added to the deletion set.
`ProjectionStore.ts:1283-1290` excludes the entire supplied set from surviving readers, also excludes existing tombstones and the selected thread, and still computes candidates through `shownBy(input.threadId)`. Live family readers outside the deletion set remain protective, including archived readers. The new argument reaches this helper through the store method. The runtime-layer spy now forwards it. Project deletion remains a sequence of separately committed thread deletions and correctly retains the default single-thread behavior.
Independent probes in `review-r2-extra-probe.log` establish:
- The previous R1 helper, extracted directly with `git show 8a414ed...:.../ProjectionStore.ts`, produces empty cleanup plans for both readers despite receiving their atomic deletion set.
- Final SQL and helper select the shared file in both plans when only the parent and side chat read it.
- An archived surviving fork that inherits the shared file protects it from both plans even when both direct readers are excluded. Tombstoning that fork restores cleanup in both plans.
- Without a cascade argument, the other live reader continues to protect the file.
## Regression and checks
The existing four-row `SQL retains fork message and answer files across deletion order` case now creates a side chat that resends `source-2-file`. It computes both plans before applying either tombstone, so it exercises the actual failure ordering. Its unchanged expected union requires that shared ID to be collected. Other live forks still protect their inherited `source-1` files and answers; native and imported histories retain their previous expectations.
Independently ran `vp test run apps/server/src/orchestration-v2/ProjectionStore.test.ts -t 'SQL retains fork message and answer files across deletion order'`: 4 passed, 52 excluded by the name filter, exit 0. Output is `review-r2-regression.log`.
Inspected supplied `r1-regression-head.log`: all 4 extended cases fail with `source-2-file` missing on the previous code. Inspected `r1-regression-fixed.log`: all 4 pass. I did not swap the shared source back or rerun old Vitest; the independent old-helper probe confirms the specific missing-file cause without editing the worktree.
Inspected supplied `review-fixes-focused.log`: 8 files, 205 tests pass. Inspected `review-fixes-check-changed.log`: format, lint, both knip passes, GraphQL generation check, affected typechecks for 3 packages, and 8 files with 218 tests pass. These are supplied fixed-tree checks, not independent R2 reruns. Independently ran `git diff --check 8a414ed... HEAD`: pass.
## Limits and handoff
No unresolved R1 finding. No new regression found in commit `844c0e3` within the requested scope. The supplied probes use the actual SQL templates and pure helper, not the live cleanup worker or filesystem. Final-head runtime and client evidence remain with the parent as requested. No client/device or full-suite verification was performed here. Cross-family explicit-ID reuse in #23703 and the pre-existing family scan were not expanded.
PR https://github.com/pingdotgg/t3code/pull/26087 remains on remote head `206166eb063a283a18446db33504b23a1fb4beb6` according to the supplied handoff, not a live R2 GitHub audit. This R2 clearance applies to local candidate `844c0e3ab436fde268786fa68dce5e5a4a9cc7d8`; the parent still owns publication, final runtime evidence, and the live PR ready check. Review artifacts are retained in the parent's requested task folder for handoff.

Unverified at this head: native Android, iOS, packaged desktop, hosted web/relay/tunnel and Windows filesystem deletion. Android Device panel opening failed twice, so no alternative device driver was used. The changed selection and locking are shared server code. Explicit attachment-ID reuse across unrelated families remains the existing issue #23703.
Implemented by Claude Opus 5.5 in T3 Code. Reproduced, tested and independently reviewed by GPT-6.1-sol through the Codex harness in T3 Code.
Macroscope's review found this PR requires human approval — The specified comparison changes OpenCode’s default text-generation model from openai/gpt-5 to opencode/big-pickle and exempts ffi-rs from Knip dependency diagnostics. Both changes require explicit human review.

The change updates rendered-page attachment selection across fork families. It also serializes fork and deletion planning, rejects forks from deleted source threads, and adds tests for attachment cleanup and concurrent operations.
Attachment selection accounts for rendered pages visible through fork ancestry and excludes pages displayed by another non-deleted family member. Tests cover source, fork, and nested-fork deletion orders. The attachment-store comment describes published IDs and inherited source IDs.

Deleting a project while a fork of one of its threads is being created can leave that fork live inside a deleted project, with its inherited pages kept on disk. The window is narrow but the leftover state is not self-healing. The snapshot and cascade should be coordinated with fork creation before merging.

Explanation
The implementation satisfies [#26076]. ProjectionStore.getThreadAttachmentIds evaluates the fork family, fork run cutoffs, deleted members, HTML render pages, and captured MCP App pages. It retains a page while another live family member shows it and returns it for cleanup when no live member shows it. The SQL regression test covers nested forks, cutoff boundaries, both deletion orders, HTML pages, and MCP App pages. The shared deletion lock coordinates fork and deletion planning through commit. Forking a deleted source is rejected.

real chrome web client, same saved fixture on main and this head: fork the first response, delete the source, and fully reload the fork. main loses inherited HTML and MCP App pages. this head preserves both, removes the source's post-cutoff page, and removes the retained files when the final fork is deleted. source runs and pages were seeded through the production event sink; fork and deletion used the real client commands. no paid provider turn was needed for this provider-independent cleanup.

Follow-up Comments
Coordinate the project snapshot with fork creation.

Coordinate the project snapshot with fork creation.
ProjectService.deleteChildThreads reads the project snapshot before it acquires withDeletionLock. thread.fork acquires the same lock through planning and commit. A fork can commit after the snapshot and remain absent from the cascade. The subsequent project deletion can leave that fork live, and a retry skips the cascade because the project is already marked deleted.
Coordinate the snapshot and cascade with fork creation while preserving the lock order documented by ThreadCommandExecutor: acquire each thread lock before the deletion lock. Add a regression test for fork creation between the snapshot and cascade.
The correction belongs in apps/server/src/project/ProjectService.ts, not at Orchestrator.ts:441-448.

ran vp i and background vp run dev separately on github/main at 6575ce32508bd7908444241f3719aa33aaf77f90 and the exact pr head. only one dev server ran at a time. drove the actual web client with the repo's playwright and /usr/bin/google-chrome, at 1440×1000. each environment had separate empty state. created the project and threads through the app, pointing at a real git repository. two real codex gpt-6.1-sol turns in each environment published the html pages through html_render. no mocks, fixtures, replay provider, or database writes.
main: fork the first completed response, delete the source through its thread menu, then reload with the cache disabled. the fork says "Unable to load Inherited page". both source html files are gone.
pr: the same flow preserves the inherited page. the fork excludes the source's second page, and deleting the source removes that later file.
nearby behavior: deleting a fork first leaves both source pages available after fresh navigation. deleting the last surviving fork removes the retained inherited file.
persistence: stopped and restarted the pr server, then freshly loaded the surviving fork with cache disabled. the inherited page still rendered. its asset returned 200; the later source-only asset returned 404. after final fork deletion, both returned 404 and the attachment directory was empty.
read-only database and filesystem checks confirmed deletion had committed and the surviving fork was live. runtime transcript.
blacksmith-check ran vp i then vp test run apps/server/src/orchestration-v2/ProjectionStore.test.ts apps/server/src/orchestration-v2/runtimeLayer.test.ts: 150 tests passed across both files, including the fork-family and concurrency regressions. github's current-head rollup lists 21 successful checks, 12 skipped, and one cancelled, with no failures or pending checks.

native electron, ios, android, macos/windows host filesystem behavior, relay/tunnel connections, captured external mcp app pages, nested forks, and concurrent deletion through multiple live clients were not exercised on this linux web pass. the existing server tests cover nested forks, mcp app page selection, and concurrency; that is not claimed as live client verification. other providers were not exercised. no merge, close, rebase, force-push, label change, or unrelated pr action.
Merged main into this branch without conflicts at ee36595. The PR's source diff remains unchanged.
Blacksmith vp run check:changed 1854e14d0943c9c80913d797bc8fa7eafc765e9c passed: format, lint, knip, GraphQL, typechecks for 3 affected packages, and 195 tests across 8 files. These include fork-family page cleanup, both fork/delete orderings, and project deletion. Two independent read-only reviews approved this head.
The fork/deletion finding is already implemented and its GitHub thread is resolved. The project snapshot/fork race mentioned in the review summary also exists on main: deleteChildThreads snapshots children before the cascade, while fork creation takes only the target thread lock. Coordinating project creation/deletion would be a separate fix; this PR preserves its scope.
merged current main into the updated head without conflicts at 206166eb063a283a18446db33504b23a1fb4beb6 and pushed without force. the seven-file change keeps its scope. the updated contributor commits already include the side-chat cleanup correction and take the deletion lock after the parent and side-chat locks.

Forks show their source's message and question-answer attachments without
copying them, so deleting the source removed files a fork still displays.
Attachment selection now uses the same family reachability as generated
pages: a thread's files are deleted only when no live thread in its fork
family shows them, with each fork edge's own run cutoff and runless rows
inherited only from imported history. Deleting the last reader reclaims
the retained files.
Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>
Deleting a thread together with its side chats planned each thread's file
cleanup while the others still counted as live readers, so a file both sent
was never removed. Attachment selection now takes the whole deletion set and
excludes it from surviving readers. The family transcript reads also use
CROSS JOIN so SQLite searches by thread_id instead of scanning every
message and turn item when it has no planner statistics.
