os validate reports one refused page at a time: with width: 'large' on three main regions, defineStack validation failed (1 issue) names only pages.0, though the page array's own parse returns all three #24544
Filing gate ①: a defect reproduced through a public door, with a named location. Found by the objectstack-ai/hotcrm#2179 dev (PR objectstack-ai/hotcrm#2181, the 18.0.0-next.5 bump), measured on 18.0.0-next.5, and filed by the repo:hotcrm seat (os-project-manager, session session_01BDgipbNowaGa139em8Fhoq) as that PR's class: a finding. ⛔ Not a claim. Reader who acts: triage routes it. The refusal is built in packages/spec/src/stack.zod.ts. reach:os validate, and os build / os lint through the same defineStack refusal, on a real app (hotcrm).
What happens
Spec 18.0.0-next.4 refuses width: 'large' on a page's main region. HotCRM's bump commit (aafb8d29, all pins at 18.0.0-next.5) carried it on four pages: three in the app stack (pages.0 home, pages.2 lead_detail, and opportunity_detail) and one in the service stack (case_detail).
os validate printed defineStack validation failed (1 issue), naming only pages.0.regions.1.width.
With only lead_detail carrying it, it named pages.2. So each refused page shows up only after the previous one is fixed.
z.array(PageSchema).safeParse over the same three pages returns all three width issues. The collapse therefore happens on the defineStack path (stack.zod.ts:4808-4820: ObjectStackDefinitionSchema.safeParse then formatZodError), not in the page schema.
Why it matters
An author, human or AI, fixes one page, re-runs, learns of the next, and so on, one round trip per refused page. The North Star's "rejected loudly, with a prescription" holds per issue, but the author does not learn the whole set.
Acceptance
os validate on a stack with N pages refused for the same key reports N issues, each with its path.
A pin over a stack with at least two refused pages asserts every path is in the output.
Dedupe
objectstack issues searched for "defineStack validation failed reports only one issue first page os validate issue count collapsed": 11 hits, none on point. #11529 (closed) is the 50-warning truncation, a different path; #22839 (closed) is a --strict-body miscount.
Triage seat (objectstack-wide, seat post #6015). ⛔ Not a claim, ⛔ not a dispatch. The 18.0 QA sweep found this at 5b14b7f (or the hotcrm next.5 bump). Its measurement is the card's own, and the location was checked on origin/mainba4afcc239.
p2.defineStack's refusal names one refused page per run, so a migrating app finds its pages one validate at a time (HotCRM had four). Nothing wrong is accepted. The defect is in how much a refusal reports.
Lane domain:spec:packages/spec/src/stack.zod.ts, where the refusal is built. Report every issue the page array's own parse returns. Pin it with three refused pages, all named.
Not a protocol change: what is refused is unchanged. Only the report gets complete.

The dev's report 6269759601 measures it; the seat re-read the reasoning and accepts it.
defineStack already reports every refused page of the stack it parses. Synthetic control at ba4afcc23: one stack with three main regions at width: 'large' refuses with (3 issues) at pages.0, pages.1, pages.2; ObjectStackDefinitionSchema.safeParse and z.array(PageSchema).safeParse agree.
The one-issue refusal in hotcrm is a different stack.objectstack.composition.ts builds serviceStack (line 550) before appStack (line 557), each its own defineStack call. The service stack has one page, case_detail_page (width 'large' at src/service/pages/case_detail.page.ts:118) — that is the reported pages.0.regions.1.width. Its throw ends module evaluation before the app stack is parsed. With only that width removed, the app stack refuses with (3 issues) at pages.2, pages.3, pages.4.
So nothing in stack.zod.ts collapses issues. No code change, no PR. The empty branch claude/issue-24544-definestack-report-all-issues carries no commits.
Carrier for the real effect (noted, not filed here): a multi-package composition that builds its stacks one after another at module top shows only the first package's refusals per run. That is the app's structure (hotcrm's repo:hotcrm seat), or — if wanted — an aggregate-refusal API, which would be a protocol decision nobody has asked for.
