Thanks for creating this issue! It looks like you may be using an old version of VS Code, the latest stable release is 1.89.0. Please try upgrading to the latest version and checking whether this issue remains.

Decorations are tracked using the default editor logic which is best-effort. It's expected though that extensions update the location and presence of their tests eventually which will reset the decoration positions based on what the extension tells us. I would expect the Python extension would do that after modifications to the file.

🤖 Thanks for the report and screenshot. Could you confirm whether this still reproduces with the latest VS Code and Python extension, and whether saving the file after undo and waiting for test discovery to finish corrects the checkmark? If it remains, please include a minimal test file, the exact save/run sequence, and the Python output-channel logs covering discovery after the undo. This issue will automatically close after 30 days if no additional information is provided.
