Efficient calculation of payoff derivatives - #1165

The HP equation system's Jacobian is now filled by index from one GetPayoffDerivBlock call per player, rather than through a Gradient method on each equation that queried each derivative pairwise. The per-equation Gradient methods are removed.

This adds a number of improvements to efficient bulk computation of derivatives of payoff functions.
