correct formatting / typos

Clean up hashgen documentation and mode descriptions for better consistency in -help and README.

• Fix typos and formatting issues in hashgen.go and README.md
• Reorder modes by hash family and keep README/help ordering consistent
• Standardize canonical mode aliases and Hashcat mode formatting
• Remove redundant hashcat compatible wording where the numeric mode already makes that clear
• Support both dashed and non-dashed mode aliases
• Fix redundant saltRaw == nil check in -m 400 phpass handling
• Remove duplicate/redundant newlines and other whitespace issues that can trigger CI warnings
