No description provided.

`tta` now also takes a sequence of flip-axis tuples, for example
`[(), (3, 4)]` for the original plus one copy flipped along the
anterior-posterior and inferior-superior axes. `True` still averages all
8 flips and `False` turns TTA off, so existing calls behave as before.

Also fixes the axis labels in the flip-table comments: after RAS+
reordering axis 2 is left-right, 3 anterior-posterior and 4
inferior-superior.
The PACS container averaged all 8 flips. It now averages 2 passes: the
original and one copy flipped anterior-posterior and inferior-superior
together, the same flip as the nnU-Net container. On the 344 held-out
cases this had as few or fewer false positives than the other 2-flip
sets and about the same as all 8 flips, at a quarter of the CPU time.

The flip set is recorded in the DICOM DerivationDescription. The
single-model path needs fastMONAI 0.11.0 for flip subsets and fails
instead of silently running all 8 flips on older versions.
The VS derived DICOM UIDs include the fastMONAI version, so the two
golden-UID tests now pin the version they were computed with.
