The stdio helpers connected with default client options. When the real
server cold-starts slower than DiscoverProbeTimeout (5 s), the client gives
up on server/discover and sends initialize on the same connection while the
probe is still queued. The server handles both concurrently: if it reads the
probe's _meta first, it rejects initialize ("Method 'initialize' is not
available on protocol version '2026-07-28'"), which is the intermittent
windows-latest failure on develop and #203; otherwise it silently negotiates
2025-11-25.
Pin 2026-07-28 and disable the separate probe timeout, as fe21367 did for the
in-process harness, in McpServerProcess and in the AOT smoke McpServerPair.
