Release 0.16.0: versioned revision bindings

Summary

• Reject workspace engine/lockfile version drift across CLI, server, and writes.
• Version calculated revision tokens while accepting legacy bindings without unnecessary re-review.
• Cover approvals, collection reviews, applicability, activations, retention, mappings, and occurrence history with upgrade tests.
• Add clean-install validation for main and pull requests.

Compatibility

This is a pre-1.0 minor release because published revision tokens and engine-version enforcement change behavior. The data model remains v11, and existing records are read without migration or automatic rewrites. Actual fact changes still require review.

Validation

• Node 24: npm ci; CI=true npm run validate
• Both npm pack --dry-run workspace checks; npm audit
• Fresh generated consumer using the local 0.16.0 package; npm run validate
• Browser check on the fresh consumer
