fix(acp): stop showing ACP v2 prompt echoes as a second user message - #24908
ACP v2 agents echo each prompt back as a live user_message. T3 shows that echo as a second user message. On a first turn with T3 MCP tools, the echo is the full prompt T3 sent: the <t3_code_instructions> block, the <user_request> wrapper and the <runtime_info> block.
Change
During a live v2 turn, the adapter hides the first user_message whose content blocks equal the blocks it sent, in order. It also hides later full or chunk updates with that message ID in the same turn. Other message IDs stay visible, including one with the same content, and so does a message whose joined text matches but whose blocks differ.
ACP v1, history loading, child sessions and continuation turns without a new prompt are unchanged. The match uses content because the v2 session/prompt response in the current schema has no message ID. A schema update that adds PromptResponse.messageId could replace it.
Scope and approval
Refs #24838. Focused fix for an obvious bug: T3 displays instructions it injected itself as a second user-authored message. The changed surface is server data shown in the conversation timeline. This fixes the live echo. It does not change history replay, which loads stored history through a separate path.
Verification
Ran the reporter's unmodified acplause v0.202610.2 (2f6bcb8, release checksum verified) on Linux against real T3 servers with isolated state, T3 MCP enabled, no API key, and the prompt Hello:
On main 5b35402623, the thread showed two user messages: Hello and the full injected prompt.
On this branch, it showed only Hello, with no provider-created user turn item.
Both runs then failed inference as expected without a key: No cookie auth credentials found. Only acplause was run live.
81 tests pass in apps/server/src/orchestration-v2/Adapters/AcpAdapterV2.test.ts, apps/server/src/orchestration-v2/Adapters/AcpRegistryAdapterV2.test.ts.
With packages/provider-acp/src/server/adapter.ts reverted to 5b35402623, 2 tests fail with assertion errors: "AcpRegistryAdapterV2 > reconciles a full prompt echo before acknowledgment", "AcpRegistryAdapterV2 > reconciles a full prompt echo after acknowledgment".
vp fmt --check and vp lint report no errors and no new warnings on the 2 changed TS files.
Typecheck passes for @t3tools/provider-acp, t3.

Macroscope's review found this PR approvable — This is a small ACP v2 bug fix that filters only the provider’s exact prompt echo while preserving real user messages and other protocol paths. The accompanying replay tests cover ordering and duplicate-content cases without introducing schema, default, deployment, or static-analysis changes.

For ACP v2 turns, the adapter retains the outgoing prompt and suppresses updates for a user-message ID associated with a matching prompt echo. A Claude replay test checks echoes before and after acknowledgment, equivalent content grouping, and unmatched messages.
Changes: ACP v2 prompt echo suppression
Layer / File(s)
Summary
Store and suppress matching prompt echoes packages/provider-acp/src/server/adapter.ts, apps/server/src/orchestration-v2/Adapters/AcpRegistryAdapterV2.test.ts
The adapter stores the generated prompt for ACP v2 turns. When a full user message matches that prompt, it records the message ID and skips later matching user-message updates. The replay test covers both echo orderings, equivalent content with different grouping, and unmatched messages.
pingdotgg/t3code#24838 — The PR suppresses the echoed prompt by matching content and message ID. It does not add the issue’s proposed reconciliation through a messageId field.

A narrow timing window can still display the submitted prompt twice. Record the echo ID before the work-state return; the remaining risk is bounded.
